Case study: The patient in this case study is K.P., a 48-year-old African American male who presented to our Primary Care Office for a three-month follow-up visit after being diagnosed with prediabetes. Today at the follow-up visit, his lab results revealed a Hemoglobin A1C of 6.5%, fasting plasma glucose (FPG) of 126 mg/dl, and a random point of glucose of 200mg/dl. The patient also reports excessive hunger, thirst, and frequent urination. Consequently, K.P. has transitioned from prediabetic to type 2 diabetes. In addition to diabetes mellitus, the patient also has a past medical history of obesity, hyperlipidemia, coronary artery disease (CAD), and hypertension.
Although there are many oral and injectable diabetic agents, I would treat this patient with Steglatro (ertugliflozin), a sodium-glucose cotransporter 2 inhibitor (SGLT2i), based on the patient’s diagnosis of type 2 diabetes mellitus, failed dietary control, and cardiovascular risk factors.
