The coronavirus pandemic has had a significant impact on healthcare organizations, forcing them to rapidly adopt and integrate new information technology (IT) and business intelligence (BI) tools. The sudden increase in demand for virtual care and remote monitoring has put a tremendous strain on healthcare providers, and many have had to find new ways to manage the overwhelming flow of patient data and ensure that this information is accurate and secure. One of the most notable shifts in healthcare organizations has been the increased adoption of telemedicine and remote patient monitoring (Desai, 2020, p. 564). This has required healthcare providers to invest in new IT infrastructure. These investment areas include secure communication platforms and digital health tools to ensure that patients can receive quality care from the comfort of their own homes. Telemedicine has become particularly critical for individuals with chronic conditions who require ongoing care, and for individuals who are unable or reluctant to visit healthcare facilities due to the risk of exposure to COVID-19 (Kuan et. al, 2020, p. 813).
Another important aspect of healthcare IT during the pandemic has been the use of data analytics and BI tools to manage the flow of patient data. Many healthcare organizations have turned to these tools to help them make informed decisions about patient care and to quickly identify trends and patterns in patient data. For example, data analytics can be used to monitor the spread of COVID-19, as well as to track the progress of patients with the virus (Czerwinski, 2020, p. 3). These monitoring measures assist healthcare providers to make informed decisions about resource allocation to better understand the disease’s impact on the population. In addition to these technical solutions, many healthcare organizations have also had to revise their workflows and processes to accommodate the new reality of virtual care (Czerwinski, 2020, p. 3). This has required significant investments in training and support for healthcare workers, as well as the development of new policies and procedures to ensure that patients receive the same quality of care regardless of whether they are receiving it in-person or remotely. The coronavirus pandemic has forced healthcare organizations to rapidly adopt and integrate new IT and BI tools, and has demonstrated the importance of these tools in managing patient data and delivering effective care. My first day at Rosecrance Florian. I was excited from the start of the day, since I entered and registered my name, and I was chosen to be with a group of young people whose age does not exceed 25 years. Most of them were close to my age. the day start by taking attendance. The attendance was about each person saying their name, what they are feeling today, and what their goal is for the day. Then the group dispersed, some of them started playing chess, and some of them prepared food for themselves. Half an hour later, they met again, and the psychologist came and started reading from some papers. The topics they read was about motivation, their belief in themselves, and their ability to overcome drugs g addiction. Each of them started reading a page from the book. Then they took a second break. I sat with them and we talked. Each person had a different story about addiction, how he used to buy drugs, and why he started using them. I felt they were really serious about quitting drugs. Then the Psychologist came and started some activities. they activity was throwing the ball and catching it, provided that it does not fall to the ground. And every time the Psychologist adds more ball between them to make it more difficult and more focus. The game helps in raising concentration and relieving stress. The lecture was replaced by self-care activity. They brought them masks and tools to take care of themselves and relax.

Delegating tasks to unlicensed personnel is a thin line and should be walked very carefully. Nursing school iterates and reiterates the importance of properly delegating tasks according to the scope of practice. Far too often over delegation and under delegation occurs increasing the risk of negligence on part of the RN for over delegation and burn out for under delegation. Delegation generally involves assignment of the performance of activities or tasks related to patient care to unlicensed assistive personnel while retaining accountability for the outcome (nursingworld.org). In essence, nurses are responsible for delegating tasks that does not involve nursing judgement. Nurses undergo intense schooling to gain the knowledge needed to execute the best action possible in medical scenarios. Nursing judgement is the culmination of education, experience, and insight that allows nurses to execute the best action possible on behalf of patients (Tantillo & DeSantis 2019). Unlicensed personnel should not perform the duty of the Registered Nurse related to lack of schooling and skill set to perform duty.
Nurses are often overworked in the skilled nursing facility. The nurse-to-patient ratio is approximately 1 to 25. Caring for this number of patients can often lead to nurses seeking the assistance of the certified nursing assistant to render medication to the patient while feeding. While this may seem like an uncomplicated task, the patient deserves to have signs and symptoms and possible adverse reactions explained to them. Monitoring the patient for these occurrences is also the responsibility of the Registered Nurse. Unlicensed personnel is practicing outside of his/her scope by performing this task.
Under delegation allows unlicensed personnel to shriek out on his/her responsibility while increasing the risk for the RN to burn out. Personally speaking, I am guilty of this action. I asked a nursing assistant to get an EKG on a patient while I was helping another patient. When I finished the task I was performing, my other patient was still had not received test. The nursing assistant stated she was about to perform it. I performed the task myself because I did not feel like hassling with the nursing assistant. By the end of the day, was exhausted because I had performed my duties and the duties of the unlicensed personnel.
Refusal of an assignment is just that. It is an excuse rendered to avoid performing the duties that are clearly defined in job description. For example, not emptying foley bags at the end of shift because the other unlicensed personnel does not perform this action. In the instance of refusal, upper management should be notified. Over delegating places RN at risk for losing license and this practice should be avoided. Under delegation, the most popular requires practice and guts to avoid.

Delegation is the transfer of roles for the completion of a task from one person to another while retaining responsibility for the results. Delegation in the healthcare sector ensures that healthcare workers practice what they have been educated to make the right judgments about patients. Delegation should have the right communication and direction, right supervision, right circumstance, and tasks to be delegated (“National Council of State Boards of Nursing,” 2016). Over delegation is a misuse of delegation where one transfers most if not all roles required to be completed by workers. It leaves the leader with minimal control of the activities taking place and at times workers may lack proper guidance or skills to complete the tasks assigned. As a result, they perform poorly. An example of over-delegation is: Would you write the patient records of all patients in the ward and make the presentation to the manager? I will not be at the hospital for three days. When over-delegation happens, I would ask the employee to have a plan in advance that helps him or her to complete some of the urgent tasks before taking a break from work. It will ensure that urgent and critical tasks are handled on time and also only a few tasks will be offered to other workers.
Under delegation occurs when one offers fewer tasks to workers and ends up being overwhelmed with duties. It can lead to delay in the completion of tasks as one relies on oneself to complete the tasks rather than focusing on the division of labor which can be helpful in such instances. An example of under-delegation: I do not trust any other employee with these tasks. I feel that I am the only one that can manage to do them as expected without errors. I want to avoid taking time to review the quality of tasks by other people. I would ask the employee to group the tasks based on complexity (Campbel et al., 2020). Afterward, the worker should take up the complex tasks that may need more expertise and reassign all other tasks to ensure he or she does not complete all the tasks alone.
Refusal to accept an assignment of legitimate delegation is linked to giving excuses when one is assigned a task. An example would be: I have never understood how to complete this work. Please give it to Anne as she does her best and will complete it fast. In this case, I would offer guidance to the worker on how to complete the task or ask an experienced employee such as Anne to train the worker. After the training, I would expect the worker to complete the task.

One of my experiences with a provider was negative. I went to a new primary care provider. He
noticed that my systolic blood pressure was slightly elevated in the 140s, and my BMI was 29. I
told him that my blood pressure is usually in the normal range, but it is high today because I had
one of the most stressful times at work as a nurse and in school to be a family nurse practitioner.
The PCP did not listen and stated that I needed to see a cardiologist and exercise more. I told the
PCP that I am a runner and an avid bodybuilder. The PCP could see I was in shape, and even
after I told him I exercise often, he reiterated that I needed to exercise. I told the PCP that I could
lower my BP by changing my diet and decreasing my stress, especially being only slightly high.
Lifestyle modifications, such as reducing salt to 1.5 gm per day and decreasing saturated fat,
alcohol, and caffeine intake, should be initiated first to decrease mild hypertension (Aronow,
2017). The PCP did not want to listen to my options, although he knew I was an RN and I was in
my last semesters of FNP school. In the shared decision model, there are five steps to making
decisions of care (Agency for Healthcare Research and Quality [AHRQ], 2020): seeking the
patient’s participation; exploring options with the patient; assessing the patient’s preferences;
reaching a decision with the patient; evaluating the decision. The PCP did not do any of the steps
in the shared decision-making model. The PCP made me angry enough to want to search for a
new PCP, but luckily, he changed his position to a hospitalist; therefore, I had to search for a
new PCP.

One way to approach designing an improvement project is to use the Plan-Do-Study-Act (PDSA) cycle. The Institute for Healthcare Improvement describes it thus:
The Plan-Do-Study-Act (PDSA) cycle is shorthand for testing a change in the real work setting—by planning it, trying it, observing the results, and acting on what is learned. This is the scientific method adapted for action-oriented learning…Essentially, the PDSA cycle helps you test out change ideas on a smaller scale before evaluating the results and making adjustments before potentially launching into a somewhat larger scale project (n.d.).
You might also recognize that the PDSA cycle resembles the nursing process. The benefit of gaining experience with this model of project design is that it provides nurses with an opportunity to ideate and lead improvements. For this assessment, you will not be implementing all of the PDSA cycle. Instead, you are being asked to interview a health care professional of your choice to determine what kind of interdisciplinary problem he or she is experiencing or has experienced in the workplace. This interview, in Assessment 2, will inform the research that you will conduct to propose a plan for interdisciplinary collaboration in Assessment 3.

Now for this assignment, you will put this information into practice by completing documentation and formulating a care plan based on a case study involving an Emergency Department client.
Tasks
1. Download the Care Plan Assignment document in the files section and read it thoroughly. This will provide you with instructions and a case study upon which you will base your documentation and plan of care.
2. Download the Patient Assessment Documentation and Profile Form in the files section. There are three (3) sections you will complete with the information from the case study:
a. Patient Assessment Documentation
b. Patient Profile
c. Plan of Care – you must include a minimum of three (3) nursing diagnoses.
3. Refer to the text, specifically the NANDA nursing diagnoses and the nursing process, the nursing diagnosis handbook, and the laboratory book to help you formulate the plan of care for the client.
4.Submit your completed Patient Assessment Documentation and Profile Form.
