According to Portney 2020, Evidence based practice is about clinical decision-making -how we find and use information and how we integrate knowledge, experience, and judgement to address clinical problems. A problem I encounter at work is patient identification errors, most facilities require 2 patient identifiers. While working in primary care as a triage nurse, I encountered two patients with the same last name of Parker. They both were Caucasian, male, and the first name and middle initial were the exact same. As I called for patient last name Parker both individuals got up and walked towards the check-in area, which made me aware that there must be some confusion. Once I clarified the correct Parker with the correct patient identifiers, I felt relieved. According to national center for patient safety (NCPS2022), the proper way to identify patients correctly is by usage of two identifiers to ensure the correct patient gets the correct medication. To solve this problem a bright orange name alert sticker is being applied to the patient’s chart to inform the nurse, physician, and other staff those multiple patients with the same name are in clinic to be seen today. Yes, this is working because it gives clear clarification and warns the staff of the name alert to ensure the correct patient is treated at the right time. It works because the bright orange sticker easily catches the eye and warns the staff to pay close attention to this chart. Thus, making the staff take extra steps to ensure that the correct patient is called. The goal is to create an environment of questioning and demanding evaluation that provides a basis for balancing quality with the uncertainty that is found in practice. These steps must be taken in effobcirt to improve patient care. (Portney2020)
