When i knew a patient was going to be discharged later that day i took the opportunity to educate my patient during the medication pass, this was a great opportunity to review what new medications they were starting and what they are for.

The role of the nurse as a health educator is to teach evidence-based education to patients to help them develop their own self-care behaviors that they can implement to prevent illness or manage their current medical diagnosis. We also educate patients on health promotion behaviors and provide patients with resources to help themselves. We teach patients and promote behavioral changes including risk factors that can decrease the likelihood of a patient obtaining a new illness or worsen their disease. I think behavioral objectives can always be implemented and reiterated, such as healthy diet and exercise, these are objectives that pertain to health promotion, but can also be incorporated in a care plan. We must also keep in mind that behavioral objectives are created for patients that are willing to change certain behavior or actions (Whitney, 2018).
When developing individualized patient education, it’s important that we start off with the assessment to develop a teaching plan and identify the patient’s readiness to learn (Ashton & Oermann, 2014) . We may discover barriers to learning that the patient may have and will need to be mindful of them when creating a teaching plan. Some of these barriers may include motivation, lack of education, or lack of time (Whitney, 2018). Strategies that we can implement when it comes to educating is breaking up the teaching material, it can be overwhelming for patients to receive all new material in a short period of time. When I knew a patient was going to be discharged later that day I took the opportunity to educate my patient during the medication pass, this was a great opportunity to review what new medications they were starting and what they are for. I would also review side effects to monitor for when starting the new medications. Later during the discharge I would focus more on the diagnosis and the rest of the discharge material. Breaking up the material allows the patient to absorb more of the information and may not feel as overwhelming.
