The family practice clinic where I do my clinical usually have patients with ages between eighteen (18) to sixty four (64) years old. Currently my number of cases as of today (October 20, 2022): Pediatric-4, Teens-3, Adults-148, Gero-28, and frail elderly-1. I still have more cases to comply but i know I can be able to get more cases on the next rotation.
The most memorable encounter with my patient during the clinical course was meeting Mrs. DLR, a 66-year-old patient with H. Pylori. The patient was very cooperative with the treatment. She wants to feel better and be relieved of the signs and symptoms of hyperacidity. She stated that she is losing weight of not able to eat well due to hyperacidity and having a bloated stomach. Mayo Clinic. (2022, May 5). When H. pylori infection signs or symptoms manifest, they are frequently associated with gastritis or a peptic ulcer. They may include the following: nausea, loss of appetite, frequent burping, bloating, and accidental weight loss.
What made it more memorable, the patient was also researching discoveries for the treatment of H. Pylori. She mentioned the new medication in the market, the dual pack and the triple pack medication for H. Pylori. Hamza, Z. (2022, May 4) For the treatment of Helicobacter pylori (H. pylori) infection in adults, the FDA has approved two vonoprazan-based regimens, one with amoxicillin and clarithromycin (Voquezna Triple Pak), and one with amoxicillin (Voquezna Dual Pak). Both regimens include vonoprazan, a potassium-competitive acid blocker (PCAB), the first acid suppressant from a new medication class that gains approval in the US in more than 30 years, and antibiotics. By preventing the stomach’s acid from secreting, PCBs let vonoprazan reach pH values that enhance the antibiotic’s ability to act.
It was a timely experience because I was just able to finish doing the case study on H. Pylori as part of the requirement each week. When a student family nurse practitioner is knowledgeable about the disease and treatment, it would be straightforward to give medical interventions to the patient. My preceptor prescribed her the dual pak teatment for H. pylori, however, she did not get approval by the insurance. It will cost her $800 for the entire treatment when she will pay from her pocket. If I can see the patient again, I want to refer her to a case manager, to help her find government fundibcing to purchase the Dual Pack.
