Clinical Problem in The Operating Room
I have been working in the operating room now for three months. When nurses are performing the time out just before the procedure, I have noticed sometimes, they don’t have the team pause on what they are doing during the time out. It vital for everyone to stop what they are doing in order to prevent any mistakes such as, wrong site, wrong patient and wrong procedure.
Solution As a nurse, I make sure everyone stops what they are doing when I perform my time out. However, not everyone across the board does this and sometimes I have surgeons and other staff very frustrated on the way I circulate the operating room. Since I am a newer nurse in the operating room, I actually followed the proper chain of command by going to the charge nurse to address the problem. I have given her a solution to this problem. That is, to print the AORN guidelines and have the whole department look at it. I also printed out an evidence-based article on the importance of everyone participating in the time out. According to (ARON journal 2022), “the most recent Joint Commission sentinel event data, there were more than 3,300 sentinel events from 2015 to 2018; 440 were wrong-site surgeries.” Also, activities and conversations should be stopped when the time out is called and until it is completed. Since we are monitored my Joint Commission, it is a good idea to always be in good habit of performing time outs for when they do come to the hospital but also more importantly, to prevent any errobcirs and maintain patient safety.
