3 Differential Diagnosis
Benign Prostatic Hyperplasia (BPH): is one of the presenting symptoms of this patient. It is an enlargement of the prostate gland. It is common among older men. What happens is that the prostate gets enlarged and causes uncomfortable urinary symptoms like blocking the flow of urine out of the bladder. It can also cause problems with the urinary tract and even kidney problems. According to the Mayo Clinic, some signs, and symptoms of BPH include frequent or urgent need to urinate, Increased frequency of urination, especially at night, difficulty starting urination, weak urine stream or a stream that stops and starts, dribbling at the end of urination and an inability to empty the bladder. These are some of the symptoms stated that this 65-year-old male is presenting to the clinic (Mayo Clinic, 2022).
Overactive Bladder (Polyuria): occurs because of miscommunication between the bladder and the brain. That is, the brain is sending a signal to the bladder to frequently cause the urge to urinate. With an overactive bladder, some of the presenting symptoms are like that of our patients. Since he is urinating frequently, and he is unable to control the urge to urinate. Some other possible risk factors for overactive bladder include hormonal changes, medications, and even neurologic changes. (Fairbanks Urology, n.d.).
Lastly, Type II Diabetes can be a cause of this man’s presenting symptoms of painless urinary frequency. Type II diabetes is a metabolic disorder whereby sugar is too much in the bloodstream or sugar is not being properly utilized in the body. One of the presenting symptoms of type II diabetes is frequent urination. Therapeutic Plan of Care
Dx. And Treatment of BPH:
All treatment should begin with a thorough assessment that will help to reveal presenting symptoms and not overlook symptoms that could be termed as very important
DRE exam
Treatment can begin with lifestyle modification that includes the following:
Have patient reduce liquid intake at night
Educate patients on abstaining from alcoholic beverages, avoiding the time of diuretics or medications that cause urinary retention or frequency
Have the patient limit alcoholic beverages, especially at night to help control or prevent symptoms.
1mg of Tamsulosin PO daily to help decrease frequency. Mayo Clinic, 2022.
Dx/Tx. Of Overactive bladder
Urine test to rule out infection
Ultrasound scan to show how much urine remains in the bladder
Botox treatment
Other medications, Fairbanks Urology, (n.d.).
Dx./Tx. Of Type II Diabetes Meletus
AIC to rule in or rule out diabetes
Lifestyle modification to prevent or decrease the intake of sugars.
