Past medical history includes asthma, chronic obstructive pulmonary disease and diabetes. Upon physical examination, you notice that Tony is struggling to breathe, his respiratory rate is 36 breaths per minute and labored, heart rate 115 beats per minute, blood pressure 90/40 mm Hg, and his pulse oximetry is 84% on room air. You notify the MD. He orders oxygen at 2 L via NC and an arterial blood gas.

Professional boundaries can help guide the interactions between provider and patient as well as amongst colleagues to ensure that services and duties are performed ethically and without undue influence. These boundaries are put in place to protect patients and must be followed in order for providers to deliver care altruistically. As health care norms have shifted toward a team approach, it is not uncommon for colleagues to interact with one another, share duties, and develop long-term working relationships (Tipton, 2017). Professional boundaries differ from personal ones in the sense that professional boundaries are intertwined with the duties or tasks performed through the course of an individual’s work. Personal boundaries can apply independently of one’s profession. I do believe that under rare circumstances, crossing a professional boundary could be justified but the ensuing actions taken by both parties must be careful. Much like the justification for conscientious objection to professional responsibility, there may be a circumstance where crossing a professional boundary could be ignored by some but likely not understood or excused by most. As stated by Tipton (2017), people may take exception to the law or professional duties on the bases of the activity violating a moral judgment as long as their actions can be supported by sound ethical judgment. I think that this concept can apply to crossing professional boundaries as well. If applying sound ethical judgment and reaching a decision to cross a professional boundary, each party must take the necessary course of action to ensure that doing so is done legally, ethically, and without harm to others. If after all that is considered and an appropriate course of action ensues, I see no innate issue with electing to cross that boundary.
