As you complete your final Request for Proposal, make sure to use information from assignments completed throughout the course. Consider including attachments to help your RFP stand out among all other funding requests. For example: Logic Model Flowchart, Program Evaluation Matrix, itemized budget table, etc. Your completed RFP must be supported by scholarly literature. The completed RFP must include the following:

Capacity
Stakeholder Involvement
Program Evaluation Plan
Data Collection
Dissemination of Results/Findings
Financial Viability and Partnerships
Program Evaluation Summary
Attachments (may include Logic Model Flowchart, Program Evaluation Matrix, Stakeholder Presentation, and/or a link to your audio file)
