Overview
This project is intended to help improve your understanding of aging as a social process and the core complexities faced by the aging population. This is a group project that will encourage your exploration and understanding of your community and the degree to which it promotes aging-in-place. As well, you will have an opportunity to develop your own plan for an age-friendly community based on your observations and share your plan with your peers in the form of a poster presentation.
Purpose
This assessment is designed to help you understand the degree to which communities promote the philosophy of ‘aging in place’ while also applying knowledge of the aging process, and what services, supports, and structures advance age-friendly communities. The scope of gerontological social work responsibilities range from direct service provision to community work, to advocacy, all of which can be usefully applied to this exercise as it links micro needs to macro efforts.
Completing this assessment will help you achieve the following learning outcomes:
Recognize the values of social work (i.e., respect of an individual’s right to dignity and self-determination) and apply it to the various challenges older adults face as they navigate through the aging experience (i.e., death/dying, loss of independence, transition into health-related services).
Identify the biopsychosocial challenges older adults face and select and apply appropriate methods and interventions based on concepts and theory grounded in research.
Assess the cognitive, physical, emotional, and psychological changes of older adults and develop appropriate treatment plans as it relates to the individual experiences of the older adult population.
Define holistic person-centered practice and recognize how associated values/beliefs can enhance the aging experience for older adults and their caregivers.

1. Community Assessment and Proposal
Section One – Community Assessment
Conduct a scan of the community using the World Health Organization’s Domains of Livability; identify areas where your community is doing well in terms of moving towards age-friendly and areas that could use improvement.

The community of focus for this project is: GEORGETOWN a part of HALTON HILLS, just outside of TORONTO, ON – GTA (General Toronto Area) and conduct a scan of the community using the World Health Organization’s Domains of Livability. (Community/health care, transportation, housing, social participation, outdoor spaces, social inclusion, focus: employment, communication/information).
Using these domains as your guide, ***identify areas*** where your community (GEORGETOWN IN HALTON HILLS, Ontario, CANADA) is doing well in terms of moving towards ***age-friendly*** and ****areas*** that could ***use improvement.***
Section Two – Summary of Findings/Proposal
Provide a written summary of your findings which include:
A ***description of each domain*** (focusing on these two Domains in particular that I need assistance with employment & communication/public awareness/information ) in what ways your community meets them, and/or in what ways your community does not meet them:
***For the domains the improvement section fir each donain, please provide recommendations for how this community can meet this domain. Including APA 7th edition/in-line citation’s) All information must be researched from credible sources.
****These recommendations should be grounded in literature (i.e., drawing on best practices, examples of livable communities, research).
Breakdown:
Description of Domains: Conduct a scan of the community using the World Health Organization’s Domains of Livability; identify areas where your community is doing well in terms of moving towards age-friendly and areas that could use improvement.
Domains to EXAMINE:
– employment (100 words x 2)
– communication/public awareness/information (100 words x 2)
1) Describe what the world health organization has set out as guidelines in these two domains for a community to meet, essentially define what they mean in these two areas. How does Georgetown in Halton hills, ON, CAnADA currently meets these guidelines (providing reliable sources/in-line citations as per APA 7th edition requirements)
2) Now define and explain how Georgetown in Haltons hills, Ont is curently meeting these guidelines 2a) (Pros/Doing well) set out by the World Health Association and 2b) Areas in need if Improvement (Cons/Needs Improvement) are they not meeting the requirements) & 2c) than suggest potential solutions on how they can improve these areas in the domain to meet the World Health Organizations requirements to meet each domain.
By 2030when their will be double the aging population due to amount of baby boomers gejerstion that will be reaching 65+ seniors age. Are they prepared?

3. Poster/PowerPoint Presentation (Oral)
Develop a poster presentation that summarizes your finding and proposal for your age-friendly community.
Posters, PowerPoint presentations/Prezi’s will be presented during a synchronous virtual discussion on a week determined during the first week of class. Two time slots will be provided to accommodate varying schedules, however -it is mandatory that all students attend at least one session, and all group members attend the same session.
For the poster presentation, you have creative freedom!
Section Three: Written Script/Microsoft PowerPoint Presentation & Poster:

1a) Write a Script to SUMMARIZE THE TWO DOMAINS WE DESCRIBED UP ABOVE: Clearly addressing researched and reliable sources of information as to how these dimsins (break down into separate summaries!
(The 2 DOMAINS: ie. SUMMARY OF *EMPLOYMENT* and Communication/public awareness/information in GEORGETOWN, Halton Hills Community.)
1b))) Now break each section into their own sections!
Eaxh section will SUMMARIZE in a script form: (to be presented by group participant) wThe findings of Section OnE & Two
1c)) break each Summary Script for each Domain into 3 paragraphs: 1) DESCRIBE each domain: how it applies to Georgetown and 2) address areas that they’re doing well (PROS) and 3) areas that they need improvement on (CONS)
